如出现索赔情况请提供以下文件材料：

1、  索赔函及清单(Claim Letter and Statement of Claim)。

2、  保险单或保险证书正本(Original Policy/Certificate of Insurance)。

3、  提单/运送契约正本(Original Bill of lading/Contract of Carriage)。

4、  商业发票(Commercial Invoice)。

5、  装箱单副本(Packing List) 。

6、  货物在装货地被装进货柜时的点算或装柜报告（Tally Sheet regarding stuffing the cargo into the subject container at port of loading or in the warehouse of the factory）

7、  集装箱设备交接单（Equipment Interchange Receipt）

8、  保险公司指定或认可之公证公司的检验报告正本。(Survey Report)

9、  运送人或其它有关方出具之破损证明文件正本(Damage Report/Exception List Etc.)。

10、被保险人对运送人或其它与货损责任有关之人(如仓库管理人、货柜场管理人)所为货损索赔一切往来函件及对方回函副本 。(Correspondence Exchanged with Carriers and Other Parties Concerned Regarding their Liability for the Loss or Damage)。

11、其他所需之相关文件(Other Relevant Document)。
